Adds group-based visibility controls to document page categories and their contents.

document.page: Categories expose a visible-to many-to-many field to choose which user groups can view the category and its children.
document.page: Child categories and pages inherit parent visibility groups through a computed list that merges direct and ancestor assignments.
security: Record rules limit access to categories and content pages to users in the allowed groups or to items without any group restriction.
